Access Health CT is Connecticut’s official health insurance marketplace. It was established to meet the requirements of the federal Affordable Care Act. Access Health CT is where you can shop, compare, and enroll in quality healthcare plans; and is the only place where you can qualify for financial help to lower your costs, or be eligible for free or low-cost coverage through HUSKY Health (Medicaid/Children’s Health Insurance Program).

The Agency for Persons with Disabilities (APD) works with local organizations and private providers to support people who have developmental disabilities and their families in living, learning, and working in their communities. APD identifies the service needs of people with developmental disabilities, and those individuals may receive social, medical, behavioral, residential, and/or therapeutic services. 

The agency serves more than 60,000 Floridians with the following disabilities:

The Children’s Health Insurance Program — or CHIP — is Pennsylvania’s program to provide health coverage to uninsured children and teens who are not eligible for or enrolled in medical assistance. Regardless if the reasons your children might not have health insurance, CHIP may be able to help you.

How do I know if my child qualifies?

Based on your family size and income, your child or teen may be eligible if they meet the following requirements:

• Under 19 years of age
• A U.S. citizen, U.S. national or qualified alien
• A resident of Pennsylvania
• Uninsured and not eligible for Medical Assistance

Is there a cost?

Most families receive CHIP coverage for free. Households with incomes above a certain level can receive comprehensive, quality CHIP coverage at a low cost.

Children’s Community Health Plan is now Chorus Community Health Plans. The Centers for Medicare & Medicaid Services, CMS, is part of the Department of Health and Human Services (HHS). On the Health Insurance Marketplace site, you’ll find information about assister programs and tools to help existing and new Health Insurance Marketplace consumers.

The Health Insurance Premium Payment (HIPP) Program is a MO HealthNet Program that pays for the cost of health insurance premiums for certain MO HealthNet participants. The program purchases health insurance for MO HealthNet-eligible participants when it is determined cost effective. Cost effective means that it costs less to buy the health insurance to cover medical care than to pay for the same services with MO HealthNet funds.

The HIPP Program cannot find health insurance policies for MO HealthNet participants, rather it purchases policies already available to participants through employers, former employers, labor unions, credit unions, church affiliations, other organizations, or individual policies. The HIPP Program is a voluntary program.

Medicaid is South Carolina’s aid program by which the federal and state governments share the cost of providing medical care for needy persons who have low income.

Healthy Connections is South Carolina’s Medicaid program. It is a medical assistance program that helps pay for some or all medical bills for many people who cannot afford medical care. The program also assists individuals who are over 65 or have a disability with the costs of nursing facility care and other medical expenses. Eligibility is usually based on applicants’ income and assets. Healthy Connections is administered by the South Carolina Department of Health and Human Services (SCDHHS) and pays medical bills with state and federal tax money.

Many groups of people can be covered by Healthy Connections. Eligibility is determined by several factors, which may include your age; whether you are pregnant, have a disability; income and assets; and whether you are a U.S. citizen or a qualified alien.

If you or someone in your family needs health care, you should apply for Healthy Connections coverage even if you are unsure of your eligibility.

Can I still have Medi-Cal as an IEHP Member?

If he or she is part of IEHP’s Provider Network, you will be able to continue to get care from the same Doctor. You can check this by calling IEHP Member Services at 1-800-440-IEHP (4347), Monday–Friday, 7am–7pm, and Saturday–Sunday, 8am–5pm. TTY users should call 1-800-718-4347.

If you also have Medicare, you can keep seeing your doctors whose services are provide by Medicare.

Tennessee’s Katie Beckett program is for children under the age of 18 with disabilities or complex medical needs. This program is for children who are not Medicaid eligible because of their parent’s income or assets. This program has two parts, A and B, and will provide services and/or help pay for medical care that private insurance does not cover.

Katie Beckett Part A

• Part A helps children in Tennessee with the most significant disabilities or complex medical needs.
• A child must meet “institutional” level of care. That means they would qualify to receive care in a medical institution like a hospital or nursing home but want care at home.
• Children in Part A receive full Medicaid Benefits and also can get up to $15,000 in nonmedical services called home and community-based services.
• A child must have private insurance, and a premium may be required based on the family’s income.

Katie Beckett Part B

• Part B is for children in Tennessee who have disabilities and complex medical needs who do not qualify for care in a medical institution. They meet “at risk” level of care. Children who qualify for Part A can also receive Part B if there is no slot available in A.
• Children in Part B do not receive Medicaid. Families get up to $10,000 a year in services to care for their child.
• Families can spend the money in Part B in any or all of 5 different ways: a card to pay for medical expenses, paying for a child’s private insurance premium, getting paid back for certain services including non-traditional therapies, hiring your own staff to provide respite and supportive home care or having a community provider for services.

Kentuckiana Regional Planning and Development Agency (KIPDA) provides regional planning, review and technical services in the areas of public administration, social services and transportation as well as community ridesharing programs. KIPDA also coordinates services for persons 60 years of age and over. KIPDA is designated by the Kentucky State Clearinghouse as the regional review agency for virtually all applications for federal and/or state funds made by organizations or governments within the state of Kentucky.

Social Services

KIPDA Social Service’s overarching goal is to support community members to remain and thrive within their communities. With this goal, KIPDA partners with community providers to implement programs to support members in their homes. These services include; Senior Nutrition (Meals on Wheels), Caregiver Services, Grandparents Raising Grandchildren, Senior Centers, Health Promotion activities, In-home services, State Health Insurance Assistance, and Medicaid Services. In addition, KIPDA provides community planning services as they relate to aging in place.

Community and Economic Development

KIPDA maintains a Community and Economic Development (CED) Division that provides technical assistance to governmental members of the seven Kentucky Counties.

Transportation 

KIPDA serves as the Metropolitan Planning Organization (MPO) for the Louisville/Jefferson County KY-IN urbanized area and is also responsible for the Regional Transportation Council (RTC). The MPO consists of Jefferson, Bullitt, and Oldham counties in Kentucky, and Clark and Floyd Counties in Indiana. Our Regional Transportation Council consists of Shelby, Spencer, Henry, and Trimble counties in Kentucky.

The Bluegrass Area Agency on Aging and Independent Living (BGAAAIL) promotes and provides for the development of community based systems of care which include: planning, access and delivery of services, coordination of activities and programs, and advocacy on behalf of and education for older persons, disabled individuals and caregivers in the communities of the Bluegrass region. Our vision is to maintain or improve quality of life and enable older persons, disabled individuals and caregivers to thrive in their chosen environment with mutual respect, dignity, and self-determination.

Medical Assistance (MA), also known as Medicaid, pays for health care services for eligible individuals. You must  meet income, resources, and other eligibility requirements in order to be eligible for Medical Assistance.

How to Apply

There are different ways to apply for MA. Please choose the option that suits you best. If you do not know if you are eligible, you can still apply.

1. Online: Using the COMPASS website, you can apply for MA and many other services that can help you make ends meet.
2. Telephone: Call the Consumer Service Center for Health Care Coverage at 1-866-550-4355.
3. In-Person: You can contact your local county assistance office (CAO).
4. On Paper: You can download an application and send to your local CAO. If you need help completing the application form, a CAO staff member can help you.

In Nebraska, CHIP is an expansion of Medicaid as health care coverage for qualified children who are without other health insurance and who do not qualify for Medicaid. Federally called the Children’s Health Insurance Program (CHIP), it provides the same services covered under Medicaid.

To be eligible for this benefit program, you must be a resident of Nebraska and meet all of the following:

• Either 18 years of age and under or a primary care giver with a child(ren) 18 years of age and under, and
• A U.S. Citizen, National, or a Non-Citizen legally admitted into the U.S, and
• Uninsured (and ineligible for Medicaid).

The Independent Enrollment Broker is a contracted statewide entity facilitating the enrollment process for Pennsylvania residents seeking home and community based services and supports as well as long term services and supports.

The PA Independent Enrollment Broker (PAIEB) manages the Home and Community Based Services (HCBS) Waiver application process. The first step to apply in the HCBS Waiver application process is to complete and submit a Community Referral Form. You can apply in one of these ways:

After you submit your Community Referral Form, your doctor must complete a Physicians Certification Form. The Physician Certification Form helps us find if you qualify. Your doctor must submit the form to the PA IEB in a timely manner to prevent delays in processing your application.

Pennsylvania Medicare Education and Decision Insight (PA MEDI) offers free Medicare counseling to older Pennsylvanians. PA MEDI Counselors are specially trained to answer your questions and provide you with objective, easy-to-understand information about Medicare, Medicare Supplemental Insurance, Medicaid, and Long-Term Care Insurance.

PA MEDI Counselors do not sell Medicare products but rather offer current, unbiased Medicare education to help you make the most informed choice about the Medicare options available to you.

RIPIN is a 501(c)(3), charitable, nonprofit organization established in 1991 by a passionate group of parents of children with special needs. These parents recognized that together they could provide support through sharing essential information and helping to find the resources they needed for their loved ones. This peer model continues to be at the heart of our work and has lead RIPIN’s network to expand statewide. Today we have over 100 employees, most of whom have personal experience caring for a loved one with special health care or educational needs.

The mission of the Barnstable County Department of Human Services is to plan, develop, and implement programs which enhance the overall delivery of human services in Barnstable County, to promote the health and social well-being of County residents through regional efforts designed to improve coordination and efficiency of human services, and designed to strengthen the fabric of community care available to all.

The Barnstable County Department of Human Services administers the regional office for the Cape and Islands SHINE Program (Serving the Health Insurance Needs of Everyone). SHINE Counselors are available to schedule in-person or telephonic appointments to provide free and unbiased one on one counseling for Medicare beneficiaries, their family members, caregivers and representatives.

SoonerCare (Oklahoma Medicaid) is a health coverage program jointly funded by the federal and state government. This program helps pay some or all medical bills for many people who can’t afford them. The Oklahoma Health Care Authority (OHCA) is the state agency that administers the program and determines financial eligibility for the program.

The mission of the State Health Insurance Assistance Program is to empower, educate, and assist Medicare-eligible individuals, their families, and caregivers through objective outreach, counseling, and training to make informed health insurance decisions that optimize access to care and benefits.

Hawaii SHIP, State Health Insurance Assistance Program, is federally funded by the Administration for Community Living and administered by the Hawaii Department of Health, Executive Office on Aging. Our Medicare-certified counselors provide free, unbiased, local, one-on-one Medicare counseling to beneficiaries, their loved ones, caregivers, and soon-to-be retirees. We also offer free, virtual presentations on Medicare-related topics and participate in health fairs and exhibitions for older adults and people with disabilities.

When you contact your local SHIP, a certified counselor will give you one-on-one guidance based on your unique situation and needs.

TennCare is the state of Tennessee’s Medicaid program. It provides healthcare to mostly low-income pregnant women, parents or caretakers of a minor child, children and individuals who are elderly or have a disability. To get Medicaid, you must meet the income and resource limits. You can apply anytime for TennCare.

There are several different groups of people that may qualify. And, each group has different income limits. Some of the groups also have limits on how much you own-your “resources.” These are things like bank accounts, cars, and land. The number of people who live in your household count too.

Some of the groups TennCare Medicaid covers are:

• Children under age 21
• Women who are pregnant
• Parents or caretakers of a minor child (The child must live with you and be a close relative.)
• Individuals who need treatment for breast or cervical cancer
• People who get an SSI check (Supplemental Security Income)
• People who have gotten both an SSI check and a Social Security check in the same month at least once since April 1977 AND who still get a Social Security check

A person who:

• Lives in a medical institution, like a nursing home, and has income below $2,742 per month
• Or, gets other long term care services that TennCare pays for

Programs and Services

Maryland’s Autism Waiver allows eligible children and young adults with Autism Spectrum Disorder to receive waiver and Medicaid services to support them in their homes and communities. The Coordinating Center is contracted by local school systems to provide case management for their students on the Autism Waiver in Dorchester, Harford, Howard and Worcester counties.

DDA Services: The Coordinating Center partners with the Maryland Developmental Disabilities Administration (DDA) to provide Coordination of Community Services (CCS) for people with intellectual and developmental disabilities and their families. Our expert Coordinators can help you plan for the future, and meet your personal goals.

The Family Resource Fund provides clients of The Coordinating Center with financial assistance to purchase items or services that are not covered by Medicaid or Medicare. The fund accepts the following types of requests:

• Critical/Safety Need- fund allocation limited to $500 or less for current fiscal year
• Housing stability (rent, security, deposit or utilities)- fund allocation limited to $500 or less for one-time only
• Quality of Life- fund allocation limited to $300 or less for current fiscal year

Home and Community-Based Services: The Coordinating Center provides supports planning services to older adults and individuals with disabilities who live and want to remain in the community or are living in a Maryland nursing facility and want to move to a home.

The Coordinating Center works in a cooperative arrangement with Montgomery County, Maryland’s Department of Housing and Human Services (DHHS) to provide service coordination and supportive services to formerly homeless individuals through the Housing Initiative Program (HIP).

The Coordinating Center assists with the intake and eligibility process for the Model Waiver. Currently there is a prioritized waiting list and applications are processed in the order they are received. Model Waiver is a Maryland Medicaid program, which allows medically fragile individuals, before the age of 22 years to live at home. Without this benefit, these individuals would be hospitalized, as their medical needs require hospital or nursing facility level of care. Under the Waiver, the parents’ income and assets are waived during the financial eligibility process even though the child continues to live in the community with their parent(s).

The Rare and Expensive Case Management (REM) Program is Maryland’s, case managed, fee for service alternative to HealthChoice Managed Care Organization (MCO) participation. The REM Program is for individuals receiving Maryland Medical Assistance with a qualifying diagnosis that is rare in occurrence and expensive to treat. The purpose of the program is to provide community-based care coordination services for people with specialized health care needs, providing access to high quality, medically appropriate health care services in a cost effective setting outside of a managed care organization.

The Katie Beckett Waiver Program is a program that allows the State of Connecticut Department of Social Services to provide Medicaid services to individuals 21 years of age and younger who have a physical disability and may or may not have co-occurring development disability, who would normally not qualify financially for Medicaid due to family income.

The purpose of the waiver is to provide home and community-based services to clients who would prefer to reside in their home or in the community instead of an alternative institution. Case management by a home health agency is provided in addition to standard Medicaid covered services such as physician services, therapy services, home health services, hospital inpatient and outpatient services.

How Do You Qualify for the Kate Beckett Waiver?

An applicant to the Waiver must meet both the medical and Medicaid financial eligibility requirements. Under the waiver, the income of a parent or spouse is not counted when determining Medicaid eligibility; however, the income must be listed.

In addition to meeting the financial eligibility requirements, the individual must meet the Department of Social Services medical criteria for institutionalized level of care. The cost of the services cannot exceed the cost of services if provided in an institutional setting.

How Do You Obtain Waiver Services?

The Department of Social Services is currently authorized to provide services to 325 clients and maintains a waiting list for those interested in gaining access to the program when an opening becomes available. To be placed on the waiting list, the applicant or representative should contact the Community Options Unit at 1-800-445-5394 or 860-424-5582.

West Virginia Children’s Health Insurance Program (WVCHIP) was created to help working families who do not have health insurance for their children. You want your kids to be healthy. One good way to keep them healthy is making sure they get medical care. Children who get good medical care have a better chance of being healthy and doing well in school.

• Are United States citizens or qualified aliens (children who are not U.S. citizens must provide verification of their alien status)
